On behalf of the Office of Student Life, I would like to congratulate you on your acceptance and welcome you to the HPU family!
This website will assist you in completing several important forms. Completing all necessary forms will require approximately 30-40 minutes. Please ensure that you have adequate time to complete each form prior to beginning. All forms must be completed in their entirety. If after completing the forms you have feedback for us about the process or the forms themselves, please utilize our feedback form located at the bottom of this page. We endeavor to constantly improve our service and, therefore, we value your suggestions.
NOTE: These forms should only be completed by students who have been admitted to Howard Payne University. If you have questions about your admittance status, please contact the Office of Admission at (325) 649-8020.
Enrollment Deposit
Information: $200 total, $100 tuition deposit and $100 housing deposit. If living off-campus, only $100 is required for the tuition deposit.
Time Required: Approx 3 min.
Housing Application/Agreement
Information: Needed to apply for residence hall housing. Eligible transfer students interested in on-campus apartment housing should contact the Office of Student Life.
Time Required: Approx. 15 min.
Student Health Form (online version) Click Here for Print Version to Mail or Fax
Information: This form is required for all HPU students and will be maintained by the University Nurse. Certified shot records must accompany the form, but they can be mailed to HPU (University Nurse, 1000 Fisk Street, Brownwood, TX 76801) or faxed (325-649-8819). Please review the health form carefully for required vaccinations.

Information: This upload tool can be used to submit scanned or otherwise digitized copies of your shot records. The records must be validated by a physician, public health clinic, or transcript from school records. Please take note of the following requirement regarding the state-mandated bacterial meningitis vaccination requirement:
- Per Texas state law, new students enrolling at HPU beginning in the Spring 2012 semester--and former students who are returning to HPU after an absence--must be vaccinated against bacterial meningitis prior to scheduling classes. A hold will be placed on the student's records preventing class scheduling until proof of the vaccination has been supplied to the University Nurse.

Request for Permission to Live Off-Campus
Information: This form is required for students with less than 4 semesters of actual college (or 60 credit hours) who will be living at home with parents and commuting to HPU.

Jacket Journey 2013 Registration
Information:
All first-year students are required to participate in Jacket Journey,
HPU's new student orientation program, and transfer students are
strongly encouraged to participate, too. Click on the link above to

January Jacket Journey 2014 Registration
Note: The site and registration page for January Jacket Journey 2014 will be updated in November 2013.
Information:
All entering students are strongly encouraged to participate in January
Jacket Journey, HPU's new student orientation program for students
beginning their HPU experience in January. Click on the link above to
